Registration Agreement

​

• Fill out registration form and check agreement box

• Make your $500 deposit or full payment (full payment due 2 weeks prior to class to guarantee spot)

• When Paying by Credit Card a  2.9% convenience fee will apply

• Proof of Bloodborne Pathogens and Communicable Diseases certifications.

• FREE Classes include a mini kit.

Student Agreement

• Students must have completed the Bloodborne Pathogens and Communicable Diseases certifications.

• Students must complete required hours to receive certificate.

• Students must arrive at scheduled times and stay for the duration of the class.

• If a student has an emergency during the class, and cannot make that day, they will be able to make it up during a future session when the course is offered.

• Student will need to provide one model for class, unless traveling from out of state.  Please provide instructor with name, number & email so they can be registered.  They will either register for a Thursday 1:15 pm time slot or a Friday 10:15 time slot.

Payment Agreement

• All payments are non refundable, however, payments are transferable to another course.

• A $500 non refundable deposit is due upon booking course to hold your spot.

Cancellation Policy

​​

• Deposits are nonrefundable, but they are transferable to other course dates.

• All payments are nonrefundable; however, payments are transferable to a different course date.

• Cancellation must be made 2 weeks prior to class date otherwise you forfeit your deposit.  This allows us time to fill your seat.
